Join in celebrating Scotland’s House of Care past, present and future.
Please join the House of Care programme for a half day web-based symposium to bring together past, present and future advocates of Care and Support Planning/House of Care. This is to celebrate and acknowledge the progress that has been achieved. It will also reflect on what has been learned and plan how to build on the significant legacy that the programme, and the wider House of Care community of practice, has built towards achieving person centred care and Realistic Medicine in Scotland.
Since 2015 Scotland’s House of Care programme has been supporting the adoption of Care and Support Planning. This tried and tested personalised approach enables people living with long term conditions to be in the driving seat of their care. This transformational shift has been achieved by the hard work, inspiration and enthusiasm of practice teams, trainers and project managers across Scotland.
The programme, a collaboration between the ALLIANCE, Year of Care Partnerships and Clinical Leadership has received support and funding throughout from Scottish Government’s Primary Care Division and Person Centred Team. Central funding came to an end on 31st March of this year.
This symposium seeks to plan how to build on the legacy of the programme to support further spread and adoption across Scotland and will also be of huge interest to the wider Health and Social Care community who are looking to develop the way they work with people living with long term conditions.
