In short, the framework describes what ‘good’ looks like in terms of the provision of effective, integrated community-based assessment, treatment, care and support.
- offers an underpinning ethos of how care and support should be configured to understand and respond to the needs of local people
- clarifies the characteristics that are present when integrated care and support is at its most effective
- describes the components of an effective, integrated health and social care system; and
- defines the ‘enablers’ that need to be in place if the benefits associated with effective integrated care are to be fully realised.
