Framework for community health and social care integrated services published

The document aims to support the identification, sharing and application of good practice across Scotland.

In short, the framework describes what ‘good’ looks like in terms of the provision of effective, integrated community-based assessment, treatment, care and support.

The tool:

  • offers an underpinning ethos of how care and support should be configured to understand and respond to the needs of local people
  • clarifies the characteristics that are present when integrated care and support is at its most effective
  • describes the components of an effective, integrated health and social care system; and
  • defines the ‘enablers’ that need to be in place if the benefits associated with effective integrated care are to be fully realised.
