Two new documents outline how Care and Support Planning can be a key enabler of the new General Medical Services contract.
As part of developing the ‘case for change’ for Care and Support Planning conversations to take place within a House of Care framework, the House of Care Programme has produced two documents detailing how Care and Support Planning (CSP) can be a key enabler of the new General Medical Services (GMS) Contract.
The strategic version of the document is high level information and is intended for Integration Joint Boards, Health and Social Care Partnerships and Health Boards; whereas the ‘explainer’ document that provides information on CSP and how it fits with the new contract.
The documents are intended for use and wider circulation to partners to inform local planning and implementation.
