The Self Management Team are looking for stories of self management as case studies for their Self Management Reflective Practice sessions.
The Self Management Team are looking for a range of contributions from individuals and organisations that can highlight real life examples of self management to be included in their Reflective Practice sessions.
The Self Management Reflective Practice session is aimed at frontline health and social care professionals to get them looking at the ways they support people to self manage and what support they need in place going forward in order to strengthen this. Through discussion and activities designed to encourage reflection, the training offers a space for exploration and ideas. The training has been designed with people with long term conditions and unpaid carers, and their experiences have been woven throughout the session’s activities.
Since the training launched in summer 2018 the Self Management team have delivered the Self Management Reflective Practice to over 20 teams across Scotland, including community rehab teams, third sector organisations and health and social care partnership staff.
Feedback from participants who have received the training has consistently highlighted the values of real life examples of self management in getting participants to understand the range of self management approaches that keep people in the driving seat and what their role in supporting people to self manage is.
