Scotland’s House of Care Programme is collecting a series of case studies and stories of change to demonstrate how the House of Care approach can support people with long term conditions self manage and engage with Primary Care as equal partners in their care.

‘More Than Medicine’ Renfrewshire

The Health and Social Care Alliance Scotland (The ALLIANCE), NHS Greater Glasgow and Clyde (NHSGGC) and Engage Renfrewshire worked together to deliver the ‘More than Medicine Lunch and Learn event linking GP Practice and the local third sector’ event. The event brought together General Practice staff from Cluster 5 in Renfrewshire to hear third sector and community organisations present on some of the positive work to support health and wellbeing from their projects in Renfrewshire.

Fochabers Health and Wellbeing For All Marketplace (More Than Medicine)

The Health and Wellbeing for all Marketplace event brought together General Practice staff from Fochabers General Practice, third sector and community organisations and member of the public to learn more about the local resources available to support health and wellbeing in and around Fochabers. It was also an opportunity for members of the public and the third sector to hear more about care and support planning within a House of Care framework.
