Partners and programme history
Scotland’s House of Care works with key partners at Scottish Government, Year of Care Partnership and Adopter Sites to deliver the programme
Drawing upon the experience and learning from the Year of Care Partnership, three early adopter projects in Lothian, Glasgow and Tayside developed the early work on making Care and Support Planning conversations routine for people living with one or more long term condition.
The guiding principle is for this approach to be the norm wherever a person is being supported or cared for – whether it is in primary and community care, secondary care, social care or in the third sector. It is crucial that all these services work collaboratively to support people and this model provides a powerful visual aid to enhance partnership working.
The project expanded to Mid Adopters in Lanarkshire and Ayrshire and Arran, and a further Adopter Site in Grampian has now developed, building on the learning gained so far. The current Adopter Sites are
