Jamie Dale

A full-time, specialist role, the Community Links Practitioner (CLP) has joined existing primary care teams.

Jamie works within the primary care team.  Jamie works with individuals on a one-to-one basis to help identify and address issues that negatively impact their health. As appropriate for general practice, there are no exclusion criteria or limit of contacts between patients and practitioners. CLPs work with anybody, of any age, experiencing any issue.

Jamie is based at 5 practices:

Asylum Health Bridging Team.

Kingspark Surgery.

The Khan Medical Surgery (Kingspark)

The Khan Medical Surgery (Farne Drive)

Muirside Medical Practice.
