Long Term Conditions Hebrides were awarded £60,000.00 in June 2021 to run their project for 24 months.
This project aims to grow and develop Long Term Conditions Hebrides service which makes a positive difference to the lives of those living with long term conditions by increasing self management training opportunities, offering new resources to the widest audience possible and building capacity and resilience within the organisation. The project will encourage more people with long term conditions to be in control of their own health and wellbeing by being better informed and more prepared to overcome everyday challenges. It will provide lifelong learning and support activities and resources, encourage skills and knowledge development, building empowerment and self-esteem for more people with long term conditions.
