Plus (Forth Valley) were awarded £120,000 in July 2024 to run their project for 36 months
A three year programme offering tailored transition support for young people and their families/carers consisting of 1:1 and peer support as young people prepare for transition; and an expanded range of activities for the adult service. Increased activities focussed on adults who’ve recently experienced transition, with themes for activities led by adults using the service. The project hopes to develop learning around what is effective in supporting better transition as well as fundamentally how these interventions at this time in a young person’s life can support their development in self management; learning from what adults are identifying as important themes, and the success of blocks of activities for beneficiaries,
