A full-time, specialist role, the Community Links Practitioner (CLP) has joined existing primary care teams.

The CLP works within the primary care team. CLPs work with individuals from practice list populations on a one-to-one basis to help identify and address issues that negatively impact their health. As appropriate for general practice, there are no exclusion criteria or limit of contacts between patients and practitioners. CLPs work with anybody, of any age, experiencing any issue
