A full-time, specialist role, a Senior Links Officer has joined existing primary care teams.

Sharon works within the primary care team. Sharon works with individuals from practice list populations on a one-to-one basis to help identify and address issues that negatively impact their health. As appropriate for general practice, there are no exclusion criteria or limit of contacts between patients and practitioners. Sharon works with anybody, of any age, experiencing any issue.
