Respiratory Transitions

The ALLIANCE is looking for young people who access respiratory services, and their parents, to complete a survey sharing their experiences.
If you are aged 12-20 and live with a condition that affects your lungs, please fill in our survey to share your experience of transitioning from child to adult respiratory services to inform future improvements. We are also looking to hear about the experiences of parents and guardians of young people who are coming up to this transition.
There are three surveys open, please pick the one relevant to you.
