The anthology brings together all Connected Communities case studies.
The Integration Support team is thrilled to announce the launch of the Connected Communities anthology.
Over the past year, the Connected Communities series has published these case studies to recognise, celebrate and learn from the achievements and successes of third sector organisations who support the development of health and social care integration.
In line with this, the Integration Support team has produced this anthology to bring together each and every case study published, highlighting the third sector’s invaluable contribution to the health and social care integration agenda, as well as spotlighting examples of innovation, transformation, partnership working, collaboration, authentic relationships, community involvement, compassionate leadership, and the role of lived experience.
The series reflects the diverse range of areas covered by third sector organisations in Scotland, which has seen case studies focus on organisations specialising in self management, chronic pain, community football, housing, nature connection, neurodiversity, mental health, additional support needs, refugees and asylum seekers, and more.
As a result, this anthology aims to highlight that when the third sector is valued as an equal partner across all sectors, Scotland is becoming a healthier, happier nation, where everyone can enjoy their right to live well with dignity, fairness, and respect.
